Notice of Injury or Loss

In order to report an incident or claim at NMSU the appropriate information must be obtained. Reporting Work Related Injuries and Illness

All work-related injuries should be reported to your supervisor using the Notice of Accident Form. Even if you do not seek medical attention at the time, your injury should still be reported. Reporting can help address a potential issue to minimize risk of injuries in the future. In addition, reporting will ensure you can still qualify for workers’ compensation in the future, should you need it.  

In general, you should give your supervisor written notice within 15 days after you know that your injury is connected to your job. For more information and required forms please visit the NMSU Workers’ Comp website or contact the Coordinator at 575-646-7375.

 

This form should be completed to document an incident and completed as soon as practical after occurrence, but within 30 days.

Notice of Incident (Record Only) Form
This form will be used for anyone who claims damages from NMSU. This could be in the form of personal injury or private property damage. Pursuant to state law, claims must be presented with 90 days of the occurrence giving rise to the claim.

Notice of Claim: Personal Injury or Private Property (Tort) Form
Departments must report loss of NMSU property due to theft, vandalism, fire, windstorm, hail or other accidental loss. This form should be completed as soon as practical after the occurrence but must be within five (5) days of the occurrence to allow for processing. In the event of a multiple loss, including both property and vehicles is to be submitted within ten (10) days of the initial report date.

Please take appropriate action to protect the damaged property and prevent further damage. Make reasonable temporary repairs required and keep accurate record of repair expenditure. NMSU coverage carries a $2,500 deductible for damages and a $5,000 deductible for theft. The department will be responsible for the deductible.

Notice of Claim: NMSU Property Damage or Loss Form
Departments must report loss of NMSU property due to an automobile or motorized equipment accident. This form should be completed as soon as practical after the occurrence but must be within five (5) days of the occurrence to allow for processing. Please submit 3 estimates of repair cost as soon as possible. NMSU coverage carries a $2,500 deductible for damages and a $5,000 deductible for theft. The department will be responsible for the deductible.
